Applying heat to a closed cylinder: the August 2026 Process Safety Beacon

The August 2026 edition of the CCPS Process Safety Beacon takes up a deceptively small case: a gas cylinder that was heated, over-pressured, and burst. Drawn from accidental-release reporting data, it is the kind of incident that rarely makes headlines but recurs across the industry, and its lesson transfers well beyond cylinders.

The mechanism

Heating a closed container raises the pressure of the fluid inside it. That is elementary, and it is also easy to forget in the moment — when a cylinder is being warmed to raise its delivery pressure, to thaw a stuck valve, or simply because it is cold and slow to discharge. The operator’s working mental model is “just warming it up.” The physics that governs the failure is the pressure–temperature relationship, and once the internal pressure passes what the vessel can hold, the container fails, often violently and with fragmentation.

The Beacon’s corrective is unglamorous and specific: applying heat to any closed or pressurised vessel should go through a hazard review and be governed by a written procedure, with controlled heat sources and limits rather than an improvised one.

Why it belongs in a study conversation

The reason this is worth flagging for study teams is that “apply heat to a closed system” is a textbook example of a hazard that lives in the gaps of a process-scoped analysis. A HAZOP built around the main process nodes will thoroughly examine the reactor, the columns, and the major lines — and can pass right over a cylinder on a utility skid, a sample bomb in a lab, or a drum being warmed in a maintenance bay. These are non-routine, small-inventory, human-in-the-loop tasks, and they are precisely where informal practice accumulates.

Two habits help. The first is scoping utility, sampling, and cylinder-handling operations into the study explicitly rather than assuming they are out of scope because they are not “the process.” The second is treating “more temperature” and “more pressure” as live deviations on any closed volume a person can add energy to — not only on the vessels that carry a tag number on the P&ID.
